Check your student’s Butler Tech forms to make sure the required PHS paperwork has been completed.\nPlease see an email that was sent on 9/29 for your personalized link to BT forms. \n2. Sign up for exactly what your student needs. PHS will bring vaccines based on our sign-up numbers.\nSign Up here \nPHS will offer TB testing\, flu vaccines\, COVID vaccines (if needed)\, and other needed vaccinations. \nTB Reminder: Students receiving the TB test through PHS must be present on Wednesday\, October 14 AND Friday\, October 16. PHS will return on Friday to read/complete the test.
